Case Management
Hope for the Future is an agency that serves as Guardian of the Person, Health Care Power of Attorney, and provides Elder Care Case Management services. Our staff are National or North Carolina Certified Guardians. The staff who work with Hope for the Future, Inc. are extremely knowledgeable about Geriatric resources and services. Case Management is comprehensive individualized services designed to avoid a future need for an incompetency hearing and subsequent guardianship.
Our specialized Case Management services and supports include, but are not limited to:
*Attendance and support at medical appointments.
*Coordination of in-home supports designed to promote continued private home living whenever possible.
*Consultation with medical providers and oversight of treatment.
*Seek, obtain and monitor residential placements when deemed necessary.
*End of life supports and services.
